KUALA LUMPUR — A man who was critically injured in last month’s accident at the Bukit Kajang toll plaza has died, raising the death toll from the incident to two.

Kajang police chief Assistant Commissioner Naazron Abdul Yusof said the 48-year-old civil servant, Mohd Hafizi Anuar, was pronounced dead at 11am today while receiving treatment in the intensive care unit at Hospital Kajang.

“The victim’s body will be released to the family once the post-mortem process is completed,” he said in a statement.

The crash took place on September 27 along the Kajang SILK Highway when a lorry carrying scrap materials rammed into four vehicles at the toll plaza.

A 16-month-old boy, Amir Husayn, was killed in the collision and several others were injured.

Authorities believe the lorry may have suffered a brake failure, with the impact of the crash throwing Amir from his family’s car and trapping him underneath.

Following the incident, the Land Public Transport Agency (APAD) revoked the operating licence of LDT Metal Trading, the company responsible for the lorry.

In a statement, APAD said its investigation found that the company had failed to install a Global Positioning System (GPS) device in the vehicle, a breach of basic safety requirements for commercial transport.

The revocation means the company is no longer authorised to operate or use any vehicles for service purposes. — October 5, 2025
